About the Role

Applied Healthcare Solutions, Inc. (AHSI) is seeking a compassionate and skilled Physical Therapist to join our dedicated home health care team. In this role, you’ll have the opportunity to make a meaningful impact by delivering high-quality physical therapy services directly to patients in their homes. You’ll work closely with physicians and care teams to create individualized treatment plans that restore function, enhance mobility, and improve overall quality of life.

Key Responsibilities

• Provide in-home physical therapy services based on a physician’s plan of care
• Assess patient needs and develop personalized care plans
• Educate patients and caregivers on therapeutic exercises and the use of assistive devices
• Monitor and report patient progress and any changes in condition
• Collaborate with interdisciplinary teams during case conferences
• Train and supervise Physical Therapy Assistants and home health aides
• Participate in discharge planning and make appropriate care referrals

Qualifications

• Bachelor’s or Master’s degree in Physical Therapy from an accredited program
• Current state licensure as a Physical Therapist
• Minimum of 2 years of experience, preferably in home health
• Strong verbal and written communication skills
• Valid driver’s license and insured, reliable transportation
• Current CPR certification
